Welcome! If you're a Medicare or non-Medicare patient, you can find your intake forms below. You’re welcome to fill them out electronically and print, or print a blank copy to complete by hand.

Please remember to bring your completed forms with you to your first appointment.

What should I bring and wear to my first appointment?

Please bring your ID, insurance card, and a doctor’s referral if you have one. If you’ve already completed your intake forms online, great—just arrive about 5 minutes early so we can make copies and collect any co-pays.

If you haven’t filled out the forms yet, we recommend arriving 15 minutes early (30 minutes if you're a Medicare patient) to complete them in person.

Wear loose, comfortable clothing, similar to what you'd wear to the gym. If needed, we have clean shorts available for your use.

How long are appointments?

Each session lasts approximately 45 minutes.

Where are you located?

14050 Juanita Drive NE, Suite B, Kirkland, WA (Finn Hill area)
We’re located in the Inglewood Professional Center next to Bartlett Eye Clinic and across from the QFC/Bartell Drugs shopping center.

Note: While our clinic is now listed in Kirkland, the area used to be part of Bothell before being annexed in 2011. For accurate directions, be sure to enter Kirkland into your GPS.

Do I need a referral to start physical therapy?

In most cases, you do not need a referral to begin physical therapy. However, some insurance providers do require one—so we always recommend checking with your plan ahead of time.

Please note: All Medicare and Medicare Advantage plans do require a referral or medical order for physical therapy to show that treatment is medically necessary.

The only exception is Aetna Medicare Advantage, which does not require a referral as of June 17, 2025.

What conditions do you treat?

We treat a wide range of musculoskeletal conditions, including back and neck pain, joint issues, post-surgical rehabilitation, sports injuries, vertigo and balance disorders, and more. If you’re unsure whether physical therapy is right for your needs, feel free to contact us for a consultation

What types of services do you offer?

Our services include manual therapy, therapeutic exercise, sports rehab, chiropractic care, posture correction, injury prevention, and more. We're happy to discuss your specific goals and tailor a plan to fit your needs.



Do you accept insurance?

Yes, we accept most major insurance plans. If you’re unsure whether we’re in-network, please give us a call and we’ll be happy to check for you.

What if I don’t have insurance?

We offer affordable self-pay options for patients without insurance. Contact our office for current rates and payment options.


How do I schedule or cancel an appointment?

You can call our office directly to book, reschedule, or cancel your appointment. We kindly ask that cancellations be made at least 24 hours in advance.


What safety and cleanliness measures are in place?

Our clinic follows strict cleaning protocols and safety standards to maintain a healthy environment for all patients and staff. Equipment is sanitized between each use, and we continue to follow local health guidelines.
